CVXPy visitors for array expressions.

Visitors: Index, Concat, Stack, Hstack, Vstack, Block

Lowers the array construction and indexing AST nodes to CVXPy expressions via cp.hstack/cp.vstack/cp.bmat, all of which preserve DCP curvature. Concat promotes scalars to 1-D before joining, and Hstack/Block route 2-D+ inputs through cp.bmat to match NumPy stacking semantics. CVXPy's bmat is 2-D only, so Block raises for 3-D+ tensors, which must go through the JAX backend instead.
